On a freebsd 4.2 system, php 4.0.5 / gd 1.8x were running happily.
Downloaded and compiled gd 2.0.1 no problems.
Downloaded and compiled php 4.0.6 with:
./configure --with-mysql --with-apxs --with-gd=/usr/local
