I'm getting this error from IIS/NTSP4 - but the php_ldap.dll module is right next to
the php_gd.dll module I'm loading (along with several others, but that doesn't seem to
make any difference).
What module is it looking for?

I just updated to v4.0.6 on win9x. Suddenly all my form-action scripts using
$PHP_SELF quit working, as it's empty. Anyone know how to fix this?
