you dont have output buffering on, check php.ini or add it manually to an
.htaccess.

Check your error log to see if there's anything there about why PHP is
dying.
Does it answer this right away, or take forever? If it takes about 10
seconds, your DNS could be messed up.
Can you run PHP from a DOS prompt on the same script? What comes out?
