Active FIPS mode (/proc/sys/crypto/fips_enabled => 1) on the server does
produce this behaviour.

That pg_attrdef.adsrc was already marked as "historical, and is best
not used" in 9.4: h

I've seen that happen with broken terminals, mostly some "web shell"
stuff. Ditch that, it's most likely broken in other ways, too.
