This stopped happening for me, and I happily ran with fully operational
suspend/resume for a long time using the Linux-4.1.y series. Most
recently Linux-4.1.17. However, a few days ago I upgraded from
Linux-4.1.17 to the 4.4.y series — currently Linux-4.4.1 — and now the
same symptoms happen again:

for me with Ubuntu 14.04.1 with (my own locally-compiled) kernel 3.18.1

The workaround no longer works since the tpm_tis module fails to load in
kernels 3.13 and newer. This means that the only way to make 14.04
Trusty to work is to apply the workaround listed here and downgrade the
kernel to 3.12.

i've been having this problem as well.
it's reproducible every single time, on the second resume.
first suspend/resume works fine.
second suspend works, resume fails.
