No can do, I'm afraid.
As noted above, original hardware is long gone.
Issue was with a 12 x 500GB PATA RAID5 array on Ubuntu 32bit
Relevant hardware on box is completely different today
Now using 3 x 2TB SATA drives on Ubuntu 64bit
No issues like this present on current setup.

Please note that I no longer have the hardware in question, and therefore
unable to provide any additional info or test any fixes, etc.
