For what it's worth I have a similar setup (far fewer users) and have started to experience a problem with ONE of the font end machines. This machine runs AMAVIS with Postfix and delivers to the IMAP server via LMTP.

Does anyone have a mechanism for cyrus whereby certain user-id's can be limited by IP address?
For example;
UserA - local network only
UserB - specific IP only
UserC - any location on the Internet

This was at a stage when TLS was not working and one of those Gremlins was around, so, I would not be surprized if that was the cause.
