I still see it regularly. I'm fairly certain that it is easy to
reproduce. The steps I go through when I see it:
1. Connect to a VPN (cisco anyconnect for me).
2. Do work.
3. Suspend computer without remembering to disconnect from the VPN.
4. Wake up computer (usually after at least an hour).

occurred in Kubuntu. I uninstalled the proprietary drivers and freezes are
back. Clearly, my problem is related to the "nouveau" drivers.

Pulse Audio seems to selectively choose to allow me to use the speakers.
I have no problems with using head phones. A reboot, allows the sound to
be conveyed over speakers, but if I pause the music/movie then all sound
is gone - accessible via headphones only.
