I had the same problem on my Debian Wheezy box up to about two days ago.
Apparently, during an upgrade the ~/,kde.share/apps/kaffeine/xine-config file
was
replaced with a default configuration file. I located the line labeled
`#audio.driver: auto`, removed the comment, changed auto to pulseaudi
