The DPMS standard defines how to signal power saving modes to the monitor.
The xset program only checks if the display system supports the DPMS
standard and sets the required power level.

FYI, I found the problem, sort of.
I had xscreensaver rotating at 2 minute intervals. Every time a new
screensaver loaded, it came out of standby mode.

I've had DPMS problems with 4.0.2 as well, though different from what
you describe. In my case, APM seemed to conflict with DPMS. In
particular, the sequence
xset dpms force suspend
apm --suspend
did not work as expected (the second line would cancel out the effects

Try putting
Option "DPMS"
in the Monitor section of your /etc/X11/XF86Config. Though be warned
that, as others have mentioned, DPMS may do weird bad things with X4, like
crash your X server.
