On the console,
loadkeys /usr/local/share/keymaps/dvorak.map
although the location of this file seems to have changed at some point.
It used to be
/usr/lib/kbd/keytables/dvorak.map.
In X, you'll need put the appropriate file in ~/.Xmodmap.
