First of all, I do not have the exact same controller that you have
(mine is an Intel Corporation 82801DBM (ICH4-M) IDE Controller).
However, since I had the same problems when I tried to use PIIX
as a module, I hope what I write below might be helpful to you.

(You seem to have loaded it as a module, that never worked in my case. I
don't fully understand why; maybe it does not get loaded early enough

I'm running Debian SID with linux-image-2.6-686, since few days i'm
facing DMA problems after upgrading kernel to 2.6.14 (from stock).
