Anyway, I have a patch (see below) that appears to work. I'll be rolling a
small bugfix release of CMTK in the next few days if/when I get a chance.

I can confirm that cmtk[1] really fails to build from source and
most probably this is due to changes in dcmtk. Any help would be
more than welcome.
