I’m actually using the homebrew dpkg-scanpackages on OS X. I found that dpkg
adds an extra / to file paths when using anything other than the current
directory.
The Homebrew folk suggested I file an upstream bug:

I agree that this would be an excellent addition to debian as none of the
apt-cacher/-ng/proxy packages really work very well.
I'd like to see this pulled in as well.

I'm seeing this too.
The trigger appears to be a dev/cciss/c0d0 block device combined with
existing lvm partitions.
