Derek,

Very occasionally (in about 2% of the attempts) sftp works in the
chrooted environment.

Could this be a clue as to what is going wrong in the typically failing
attempts?

Are you sure it's "Connection refused" that you're getting?  If rsync
works, that doesn't make much sense.  "Connection closed" seems more
likely.  In that case, see this:
