It uses a combination of whitelists, blacklists, and confirmation messages to ensure that you only get mail from known senders and/or real human beings with working reply addresses.
A few months ago I wrote my own set of scripts to get the same basic functionality and I've gone from 30-50 spams a day to about one or two a month.
