That one's for the IA64 CPU, which you don't have.

I just checked the site you mentioned, and apparently, you missed two
entries:

Linux 2.x (libc6 i386)
Linux 2.x (libc6 i386) RPM

You need one of these (you mentioned yourself that you have an i686, didn't
you? An i686 is iX86 class and hence i386 (backwards-)compatible), probably
the "RPM" one.

It can't, as you don't have an Alpha CPU, but an iX86.

However, I have to say that form is extremely badly written - listing stuff
like "Linux/Alpha" under "OS" *and* having an additional "CPU" entry doesn't
make any sense whatsoever, so I'm not surprised at all that you were
confused. Real really made a mess out of that form.
