I'm using Tim Waugh's excellent Selfdocbook (XML Edition) release for generating documents.
Now that I have a document of reasonable size, I'm trying to generate an index. I'm not having success. I have tags such as: <indexterm id="XX"><primary>XX</primary></indexterm> throughout the document, but no index is being generated. My question is how do I go about making an index for my document?
