First, try kep in a non volatile file all data about it !!!!

Try to see all files relatives to log any action done by the kernel. This is
in /etc/syslog.conf, at his point, you must  find out any entry  like :
kern.*      /dev/console

and insert down a line as :
kern.*    /my_log_file

.... that's for capture in a file all log about it !!!!! Of course, any
error about floppy will be catch here too !!!

Second !!!! the most important !!
NEVER get out  a flopppy before umount it !!!!!
