I frequently see these things begin to boot, get mostly through the
kernel loading process, and then die with "Boot failed ... insert
another disk and press a key", or something like that. It's
invariably a bad floppy or several, or perhaps a bad floppy drive. I
just keep whipping floppy disks across the room (much to the chagrin
of my cat) until one of them works.
I create them with this:
dd if=boot.img of=/dev/fd0
As another poster pointed out, the disk imaging process does not work
around bad sectors, as would a low-level format of the floppy. This
greatly increases the chance for errors, unfortunately.
