That is a CAP based model, which is sorta non-standard. Most phone
companies are using g.dmt, the equivalent model being a 677. These do
NOT interchange.

You will probably need to be more specific as there are several
flavors of ADSL (DMT, CAP, g.lite). Ameritech is probably ADSL/DMT.
