All attempts to login to my server now generate "login incorrect" 
error msgs.  This applies to all system logins.  Passwords and 
login names should be correct..

I used a root disk to login to the system and changed the root 
password to null.  This allowed me to login as root when the 
system was rebooted.  I then used passwd cmd to change root 
password.  

Even with a new password defined, however, attempts to login as 
root in another terminal session still generate "login incorrect".

Obviously login function has gone south on my server.  

How can login best be restored?

Would this be a symptom of a hack?
