Many possible reasons - one of the most important ones is probably
"never change a working system". I personally am still on RHL 5.2 and I
won't change until at least RHL 6.2 is out. I went the same way with 4.x
-> 5.x, i.e. 4.2 -> 5.2. I simply don't trust x.0 releases and the
flurry of reported bugs which ensued on e.g. this list after both 5.0
and 6.0 wasn't exactly encouraging. Also, I'm not missing any important
features 6.x has and 5.x hasn't and I suspect that to be true for many
more people.
Fortunately, there are only few reasons ever forcing one to upgrade
(even securiy upgrades are still provided by Red Hat for 4.2 and 5.2,
which is really nice), which is one of the things I really like about
Linux.

My EUR0.02,

Thomas
