You may want to check out custodian. You can find it at freshmeat.

Over where I work, we have a standard procedure whereby all new users
with access to IT resources especially email & internet will be briefed
on the company policy regarding use of IT infrastructure & resources.
They then sign an acknowledgement form before a userid is given to them.
