Is there anything in the apache error logs? If you put a bunch of
garbage in the .htaccess file, do you get an "Internal Server Error",
or is apache just ignoring the .htaccess file (no error?).

Well, the only other thing I can think of is 'AccessFileName .htaccess'
isnt there, or got changed.
