Graham, if you don't get a decent response here I'd suggest that you check the postfix users list archives. I know this has been discussed before on that list.

After a whole lot of digging, the problem seems to be this:
Redhat's version of cyrus-sasl contains both sasl1 and sasl2 in a single package. The postfix that ships with redhat is linked against sasl1. The docs and support sasl programs are only for sasl2, so nothing works.
I am currently trying to figure out how to convince postfix to build against sasl2.
