As great as Qmail is made out to be, you'll probably find life with
Courier to be much more pleasant:

You can think of Courier as a Qmail that's actually maintained.  If you
use Qmail, you're likely to use Courier's IMAP and POP servers as
well...  Using the Courier MTA instead of Qmail just means less
configuration for some items.  Most of the MTA configuration is very
similar between Qmail and Courier.

Sendmail won't be an "open" relay if you configure it properly.  Steve's
instructions look right for that.  Sendmail will add a Recieved header,
but should otherwise leave them alone.

There's nothing that differentiates a mail server in a private network
from a "real" public mail server.  They both are going to receive and
process messages over SMTP.
