I had this card as an Iomega Zip Zoom card.  Worked well with Linux.
I ran it as a 152x and specified the parameters with a command line
at boot (the APPEND option in LILO).  It's all in the SCSI HOW-TO.  
I had support compiled into the module, and this was Slackware 3.x, but it
should work just the same for you.
