You have competing non-standard evaluation tools in play. I would decouple them
with a function (untested):
my fit <- function( DF ) {
data.frame( HOSP_NRD = .$HOSP_NRD[1]

, data=DF)
)
}
dfhosp <- ( dat.2.wide.sub
