CMS protocol require that you begin with a medical doctor's order for an evaluation by a licensed Occupational Therapist. The OT Eval, goes back to your medical doctor and your doctor issues a second order for a wheelchair that meets the evaluation specifications. That order goes to a Provider that submits the costs to CMS for approval and once the cost has been approved, the chair is built and customized to your medical needs.
