Your choice of hardware is going to depend on your ability to move.  I use a quadjoy mouse.   If all you can do is move your eyes,  they make eye gaze systems that track your eye movement.  If you just have gross arm movement, try the bigtrack.  I used it with my daughter when she was little.  You can add switches to it also.  In fact, I might still have it around here somewhere if anyone on here wants it.  http://www.infogrip.com/bigtrack-trackball.html
