briefly, register an "extension => progid" mapping under
HKEY_CLASSES_ROOT/extension
and a "program identity => command" mapping under
HKEY_CLASSES_ROOT/progid/shell/open/command
under HKEY_CLASSES_ROOT
(use regedit to see how it's done for other applications)
