I'm guessing, as I don't have that card, but I had similar problems and the 
following worked for me.

Try editing /etc/pulse/default.pa and change
load-module module-hal-detect
to
load-module module-hal-detect tsched=0

This switches off the new glitch-free mode which, due to various problems with 
buggy ALSA drivers and kernels with needlessly high latency, isn't.

There is a link somewhere which explains this, but I can't find it. It seems my 
Google finger is broken :)
