Unfortunately, some documentation had not been updated after new
features were added. ADDRESS_REWRITING_README was lagging behind
postconf(5) which did not help.
I added a new table that summarizes all the features that can affect
the determination of message delivery transport and next-hop
destination, including their precedence order.

I also updated descriptions of individual configuration parameters.
There, I think I should add pointers to the new table.
