It's most possible to perpetrate some evil hacks down in /var/lib/pkg, but why not just update the port? It's unlikely to be terribly difficult, and the maintainer (avsm at openbsd dot org) would most likely be happy to take a look and help out with any issues. You can do your work under /usr/ports/mystuff; there is quite a bit of documentation available, and updating the port might well be as simple as changing BASENAME, DISTNAME and PKGNAME in the Makefile.
