I agree the missing original and resend seems to be pretty good
evidence that locking is not the issue.  It's good to eliminate that
problem.

I haven't read this thread closely, but from what I have seen I don't
know the solution.  Your setup is somewhat complex with the various
layers.  It sounds like procmail is doing the right thing and has been
for a long time.  What is the most recent component you've upgraded or
changed?
