To be completely honest, it's slightly free-flow. I used to run some years ago (e.g https://community.kde.org/Get_Involved/Quality/Beta/4.10/Plasma https://community.kde.org/Get_Involved/Quality/Beta/4.10/AreasToTest) where I'd make a list of things to focus on and people would go yolo. We'd hang out on IRC during and it seemed a lot would get done.
