[Pixman] Several bug fixes and MIPS fast paths.

Per previous code review:
 - All commits that contain fast paths are separated so that there is only
   one fast path per commit. Also commit messages are formatted better.
 - For fast paths that don't have test cases in lowlevel-blt, these are added
   in separate commits.
 - Added more descriptive commit messages for bug fixes.
