it happened to me something pretty similar when I upgraded to mysql 3.23.54a. While solving the problem, I suggest to uninstall mysql 4.0.1 and reinstall your previous working version (unless there's a real urge to upgrade to 4.0.1) and then try to upgrade to a devoloper server reinstalling not only mysql but also php and apache. I succeeded doing that way.
