There is no "value" attribute.  But you still need to include the "name"
attribute if you want it to come back as a form variable:

        echo "<p><b>Notes:</b><br>\n<textarea rows=5 cols=31 name=\"notes\" 
wrap=virtual>" . htmlentities($value) . "</textarea></p>\n";
