In PhpStorm land we have our intentions creating it on the first line after <?php with no blank line, which seems to have gone down well with no requests (yet) to change it. If there is interest, I can try to find out how many people are using the intention to give an indication of how widespread the usage is?
Personally, I prefer this too, but in all honesty, I would rather there be consistency (whatever it is) so would take anything here.
