The only area I can see this might happen is if your History settings are set 
to create a new object (snapshot, etc.).
In that case, on my copy PS will use the name "Open" , but not "untitled".
I could not get it to change the file name in the way it does on your system.

Note, Photoshop has dropped the lettered prefix in favor of "untitled", 
and is using the numbered suffix.

It's curious. As I mentioned, if I use the Export command, it works fine.
