It sounds like the aperture is not opening up as it should when it's mounted to the camera. The 16-45 doesn't have any controls on it that would prevent the aperture from opening, so you're not doing anything wrong. With the lens off the camera, looking at the mount end (rear) of the lens with the red dot at 12:00, there's a lever at 3:00. If you push that with your finger, you should be able to see the aperture open and close. Maybe exercising it a little will help. Otherwise, I think it may need to go back to KEH, unfortunately.
