Lately, not much because I'm trying to improve my hand-held technique, but as a general rule: street photography, hardly ever, well let's say never; macro, most of the time when photographing with serious intent :-), but I find it relaxing to do evening walkabouts & take my 50mm 2.8 macro for hand-held close-ups--focus is often a little soft, but I don't care at these times--it's just play & relaxation; any semi-formal portrait sittings I do with friends & family I always use a tripod, though I'm going to photograph a friend next week & I'm going to try some hand-held shots as well as tripod shots; In general though, I would say, if there's a shot I'm really serious about, my first inclination is, in fact, to use a tripod.
