There's no "problem". Some camera and lens assemblies are heavy and/or unbalanced enough that simple friction between the plate and the body are not enough to hold them in place properly when flipped over on their side for a portrait oriented photo, they'll tend to twist. So Manfrotto offers a plate with a ridge on it that allows the camera's edge to butt up against that and help prevent twisting.
Most of the tripod QR systems offer standard flat plates and fitted plates like this, to handle a variety of needs and situations. Calling it a "problem" is exaggerating the reason for their existence. Since I use mostly lightweight lenses, I have never needed a fitted plate for the DS or K10D.
