It doesn't do it all the time but it happens often enough to keep catching me out as I keep forgetting about it (most of the time I finish with Photoshop before switching accounts).

Of course, it could easily depend on the other apps/utilities I have on my system. I tend to leave quite a lot running.

I solve this working environment problem by
  a) using gentle, soft toned B&W images as desktop backgrounds,
b) using Full Screen mode in Photoshop for editing adjustment most of the time,
      with a mid-tone to light gray backdrop.

With two screens, the full screen mode doesn't have quite the same benefit as it only works on the primary monitor. In my "work" account I chose a solid mid-grey backdrop and the "graphite" colour scheme. I find it a bit bland, which is why I have my second account with a nice big panorama spread across the two screens.

Most of the time I am using one of the two full-screen modes anyway. The other screen has the palettes and a Finder window. I tried going back to a single screen for a while, but it ended up getting in the way when I started needing lots of windows open when working on a project in Dreamweaver. F9 and F10 were the only things standing between me and insanity.
