Have you checked for some kind of contamination in the tanks and reels?

I assume both the developer and the fixer were fresh? That is the first thing I would check. Also I would try developing roll using only distilled water in all steps to eliminate the possibility of something in the water.

Then, if the problem continued, I would try using an acid stop for a roll (just to eleminate that possibiliy as I also use a plain water stop bath).

If none of those help, then the problem must be the film itself.

By the way, if using distilled water there really should be no need for the photoflow. Are you sure of the distill water source?
