there is no real forms designer available for Mac at the moment. The corporate lemmings at the major software vendors are limited to one single platform, forgetting about the IMHO best development platform out there at the moment (you guessed it... OS X).

Anyway, the best you can do at the moment is, if you have a lot of consistency in your documents (similar fields used all over the place), and also a bit of persistence and understanding of programming, to create EPS files containing the needed base structure for the fields using the PDFMARK operator. You then will place these files on your form and they will pre-define your fields. You might then only check them afterwards.

For introducing JavaScripts, you could either create some FDFs which would then fine-tune the fields, or build up a library for your own purpose.

Is there any consensus on a forms design program for the Macintosh?

I will be needing to create lots of forms from what start out as Word documents but the need for form fields, javascript validation, and calculations leads me to really want the help of a program dedicated to this purpose.

What is the most successful program for this purpose?
