As a matter of fact, I think the easiest way to achieve this is to make several combinations of form templates, and then convert the problem back into merging data and forms.
i.e., assume the data comes form somewhere, and the initial intension is
(data + form_a) + form_b
it can be converted to
data + form_c, where form_c is pre-edited, not dynamically generated, template of form_a + form_b

Do any of the fields have the same name? If so, will they always have the same value?
For this application they are named Name1 to Name4 and Status1 to Status4 8 fields in total and a hidden field with a fixed value.
The data entered will be individual to the user completing the form.
