I've downloaded and looked at both the html app and the pdf app.
Exactly what is it you are trying to accomplish? Anyone that is going to try and help you will need to know the workflow, database, routing, e-mail notification, etc.
We Do That! Technologies can very quickly and easily create a database from either of the two forms and store the incoming data, we would recommend that you use the html as you well know, it loads a lot quicker.
You could create your own HTML form using our formBuilder technology. It is in Beta right now which makes is still FREE, but when we release it the price will still only be $35/form. It gives you a _javascript_ library that helps validate entries such as numeric only, date fields, upper case, etc.
We can also host the processing of the data from your forms on our website so you don't have to worry about having a machine properly configured, messing with Perl scripts, database design, etc.

I am not good at software. I have tried to find a way for many year to post an editable form on my wifes small website for her clients to use and if you check the site I think you will see what I mean. The url is http://www.attyins.com/app1.pdf but be very patient as it takes forever to load. It is not even complete and I do not want to go further without some idea of options. The form we have been using is at http://www.attyins.com/law2.html and the uneditable copy of the app1.pdf is at http://www.attyins.com/App.pdf. I would appreciate any help anyone has to offer. I am presently looking for work so I have a bit of time to play it would just be nice to see some progress being made.
